Welcome and thank you for taking the time to complete the Enhanced Lifestyle Planner Questionnaire. The Questionnaire takes approximately 20 to 30 minutes to complete and can be completed in one sitting or in several sittings.

If THIS IS THE FIRST TIME you access the Questionnaire AND your Planner has given you the Access Code to register, begin the process of completing the Questionnaire by registering yourself. Double Click on Registration to get Logon Access and follow the instructions. If you do not have an Access Code, please contact your Planner.

Registration to get Logon Access

If you are re-accessing the Questionnaire in order to complete it or make changes to some of your answers, OR if you were already registered by your Planner, you can access your Questionnaire by selecting Logon Access to Questionaire

To get Log On access to your Questionnaire, use the Email and Password that you have selected for yourself when you previously completed your Registration or the ONES GIVEN TO YOU BY YOUR Planner when he or she registered you.
